Treatment Plan

Anticipatory medications:

• Initial subcutaneous PRN medications
• Consider switching to a syringe driver if requiring 3-5 doses/24 hours Syringe driver dosing:
• Morphine (for pain + SOB)
• Fentanyl (for pain + SOB): (if LVEF <30%)
• Other opioids that could be used: Oxycodone(if renal issue), Diamorphine
• Midazolam, Levopromazine (for agitation)
• Levomepromazine, Metoclopramide, Haloperidol, Cyclizine (for nausea/vomiting)
• Hyoscine butylbromide, Glycopyrronium (for secretions)
