Treatment Plan
Anticipatory medications:
- Initial subcutaneous PRN medications
- Consider switching to a syringe driver if requiring 3-5 doses/24 hours Syringe driver dosing:
- Morphine (for pain + SOB)
- Fentanyl (for pain + SOB): (if LVEF <30%)
- Other opioids that could be used: Oxycodone(if renal issue), Diamorphine
- Midazolam, Levopromazine (for agitation)
- Levomepromazine, Metoclopramide, Haloperidol, Cyclizine (for nausea/vomiting)
- Hyoscine butylbromide, Glycopyrronium (for secretions)
