Conservative:
• Inspect others
• Surface crust removal
• Disinfection
Medical management:
• Adults: oral antifungals (pre/post microscopy results)
• Children: oral antifungals only if confirmed and clinician specialised in
dermatological conditions
• Co-prescribe topical antifungal agents (e.g. ketoconazole shampoo) for 2-4 weeks to
prevent transmission
Oral antifungal options:
• Terbinafine: requires baseline LFTs and repeat at 4 weeks (urban areas)
• Griseofulvin: 4-8 weeks course (rural areas)
Follow up:
• Review at 4-8 weeks
• If suboptimal response: continue treatment for 2-4 weeks or resample
Referral criteria:
• Suspicious kerion
• Children requiring oral antifungals if not specialised
• Uncertain diagnosis
• Treatment failure in primary care
