Treatment Plan
Primary care management:
- Wound management
- Analgesia At <5 years with full status = No V.
- Skin care advice: cold baths, compresses, emollients
- Sun protection
- Tetanus prophylaxis assessment At 5–10 years with full 3rd course + 1
- Infection monitoring preschool booster = No V. needed but:
- Review within 48 hours If no booster = if clean wound then no
- Follow-up every 3-5 days until healed
- V. needed, If tetanus-prone wound = Clean it, give V., If very high risk, give V + HTIG at different site to V
Immediate hospital referral for:
- Complex burns If >11 years old with full immunisation
- Full thickness burns (<10 years ago) = No V. needed.
- Deep dermal burns >5% surface area
- All deep dermal burns in children If >11 years old with full immunisation
- Circumferential deep dermal burns but >10 years ago = Follow same as the
- Critical area involvement 5-10 years old with no booster (3rd Inhalation injuries bullet point)
- Significant comorbidities
- For not received courses/ unsure status = Clean wound, give V.
Prognosis
• If tetanus-prone wound or high risk of Varies by depth: T = Give V + HTIG.
- Superficial epidermal: resolves within 7 days, no scarring
- Superficial dermal: resolves within 14 days, no scarring
- Deep dermal/full thickness: requires surgery, risk of scarring/contractures
