• Start on pre-term formula upon discharge from the hospital.
• Nutriprem 2 Powder or SMA Gold Prem 2.
• DO NOT use ready-to-feed versions unless clinically needed.
• Use up to 6 months corrected age (e.g., 6 months EDD to 26 weeks).
