Treatment Plan
Angioedema:
- Rapidly progressing: IV chlorphenamine + hydrocortisone, arrange admission
- Stable: Non-sedating oral antihistamines (fexofenadine, loratadine, cetirizine)
- Pregnancy/breastfeeding: Loratadine preferred
- Severe symptoms: short-course oral corticosteroids
- Duration: 6 weeks initially, review
- If persistent: Continue 3-6 months
- If severe urticaria: Continue 6-12 months
- Short-lived symptoms: PRN
Anaphylaxis:
- Call 999
- IM adrenaline (1:1000) outer lateral thigh:
< 6 months: 100-150mcg (0.1-0.15ml)
6m-6y: 150mcg (0.15ml)
6-12y: 300mcg (0.3ml)
> 12y/adults: 500mcg (0.5ml)
Repeat every 5 minutes until improved – IV chlorphenamine dosing:
< 6m: 250mcg
6m-6y: 2.5mg
6-12 y: 5mg
> 12 y/adults: 10mg
- IV hydrocortisone dosing:
< 6m: 25mg
6m-6y: 50mg
6-12 y: 100mg
> 12 y/adults: 200mg
- Salbutamol nebuliser:
< 6y: 2.5mg
> 6y: 5mg
