Treatment Plan
B12 with neurological symptoms:
- Urgent haematology referral
- If delayed: 1mg IM Hydroxocobalamin for alternate days until the review
- If no further response: reduce to every 2 months
B12 without neurological symptoms:
- IM: 3x weekly for 2 weeks, then:
-Non-dietary cause: IM every 2-3 months or oral 500- 1000 mcg daily
-Dietary cause: oral cyanocobalamin 50- 150 mcg daily or 2 yearly IM
Folate deficiency:
- 5mg folic acid for 4 months
- Dietary advice
