TREATMENT PLAN
Types:
- Aspirin
- Clopidogrel, Prasugrel, Ticagrelor (CPT)
- Dipyridamole
- GLP2b/3a inhibitors (abciximab)
Indications:
- Secondary prevention in ACS/angina/PAD/AF (although DOACs are usually prescribed for AF)
- CAD – Aspirin LT
- Cerebrovascular – Clopidogrel LT
- PAD – Clopidogrel LT Specific regimens:
- Post-ACS: Aspirin (lifelong) + second antiplatelet (12 months)
- Post-PCI elective: Aspirin + second antiplatelet (6 months)
- Post-TIA/stroke: Aspirin + clopidogrel (3-4 weeks), then long-term clopidogrel
– If aspirin/clopidogrel contraindicated: Higher dose clopidogrel monotherapy
Medication considerations:
- Avoid omeprazole/esomeprazole with clopidogrel
- Use lansoprazole/pantoprazole if PPI needed
Pre-procedure management (don’t need to stop for dental procedures):
- Clopidogrel: Stop 5-7 days before
- Prasugrel: Stop 7 days before
- Ticagrelor: Stop 3-7 days before
