- Suspected pathological cause
- Lingering uncertainty
- Relevant family history
- Known/suspected genetic conditions
- Parental concern
PROGNOSIS
Varies based on the underlying cause
KEY INFORMATION
Common pathological murmurs:
- PDA: machine-like murmur
- Aortic stenosis: ejection click radiating to carotids, not respiratory-dependent
- Pulmonary stenosis: ejection click radiating to lungs, varies with respiration
- ASD: fixed splitting of the second heart sound
- VSD: pan systolic/early-mid systolic murmur
EXTRA’S
Still’s murmur:
- Age 2-7 years
- Vibratory/musical early-mid systolic murmur
- Louder when standing vs sitting
- Reduced with the Valsalva manoeuvre
