SIGNS & SYMPTOMS
Peripheral:
- Normal neurology
- Severe nausea/vomiting
- Possible hearing loss
Central:
- Prolonged severe vertigo
- New onset headache
- Recent trauma
- Focal neurology
- Central nystagmus
- Abnormal Dix-Hallpike
- Unable to stand/walk with eyes open
INVESTIGATIONS & DIAGNOSIS
Tests:
- Romberg’s (limited differentiation value)
- Dix-Hallpike (positive in BPPV)
- Head impulse test (Vestibular Neuronitis vs Cerebral infraction)
- Unterberger’s test (Marching on one spot with eyes closed. If there is Labyrinth
damage, then there will be lateral rotation to the affected side) - Alternate cover test (if there is any vertical correction, this suggests stroke)
- Imaging if indicated
