TREATMENT PLAN
Conservative:
- Usually self-resolves in 1-3 days
Antibiotics indicated for:
- Bilateral otitis media
- Systemically unwell
- High risk of complications
- Tympanic membrane perforation
Types of antibiotics usually used:
- Amoxicillin 5-7 days is first line, if worsening symptoms (no better in 2-3 days)can
consider co-amoxiclav - Alternative: clarithromycin/erythromycin if penicillin is contraindicated
HOSPITAL ADMISSION CRITERIA:
- Severe systemic symptoms
- Suspected complication of AOM (these include: Meningitis, Mastoiditis, Intracranial
abscess, sinus thrombus, and facial nerve paralysis) - Children 3-6 months of age
- – Temperatures >38 or more
- < 3 months of age (Have a low threshold for admission for these patients)
PREVENTION:
- Avoid smoking/passive smoking exposure
COMPLICATIONS:
- Otitis media with effusion OME (glue
ear) - Hearing impairment
- Speech/language delay
- Communication difficulties
- Tympanic membrane damage
- Mastoiditis
- Intracranial abscess
- Sinus thrombosis
- Meningitis
