TREATMENT PLAN
Conservative:
- Keep your ear dry and clean
- Acetic acid drops (7 days)
- Aural toilet if obstruction present
- Simple analgesia
Medical:
Topical antibiotics:
- Gentamicin (contraindicated if TM perforation)
- Ciprofloxacin
- +/- Corticosteroids (if no fungal infection is suspected)
Antifungals:
- Clotrimazole
- Chloroxylenol
Oral antibiotics:
- If severe infection
- If spread beyond the canal
- If systemically unwell
Referral:
- Urgent: malignant otitis externa, sepsis, systemically unwell
- Routine: recurrent/chronic cases, immunocompromised patients
