Conservative:
- Reassurance if simple cases
- Address underlying cause if secondary
- CBT if psychological impact
- Hearing aids if associated with hearing loss
- Medication review/adjustment if drug-related
Referral criteria:
Immediate (within hours):
- Suspected stroke
- Focal neurology
- Acute vestibular symptoms
- Sudden onset pulsatile tinnitus
- Post head trauma
Very urgent (24 hours):
- Sudden onset within 30 days
Urgent (2 weeks):
- Sudden onset >30 days ago – Worsening hearing – Persistent otalgia/otorrhoea
Routine:
- Persistent unilateral tinnitus
- Unilateral hearing loss
Persistent pulsatile tinnitus - Persistent objective tinnitus
- Bothersome symptoms
