Treatment Plan
If systemically unwell:
- Hospital admission required
If not systemically unwell:
Non-pregnant/catheterised/male patients:
- Cefalexin 7-10 days
- Co-amoxiclav 7-10 days
- Ciprofloxacin 7 days
- Trimethoprim 14 days
Follow up:
- Reassess within 48 hours if no improvement/worsening
Referral criteria:
- Recurrent UTIs
- Men with 1 episode without an obvious cause
- Pregnant patients
- Non-urgent: recurrent UTIs in women > 40 years
2-week referral criteria:
- > 45 years with unexplained visible haematuria without UTI or persistent/recurrent visible haematuria after treated UTI
- >60yrs with unexplained non-visible haematuria + dysuria/raised WCC
Prognosis
Complications include:
- Renal abscess
- Sepsis
- Preterm labour
- Renal scarring
- Recurrent UTIs
