Treatment Plan
Conservative measures:
• Increased fluid & fibre intake
• Simple analgesia
• Warm bath soaks
• Topical corticosteroids (1 week)
Medical treatment:
• GTN ointment 6-8 weeks if symptoms persist >1 week
Referral criteria:
• Children: If not settled within 2 weeks
• Adults: If GTN ointment is ineffective after 6-8 weeks review
• If secondary causes require specialist input (e.g. colorectal cancer, IBD)
GTN ointment contraindications:
• Age <18
• Concurrent phosphodiesterase-5 inhibitors
• Postural hypotension
• Hypovolaemia
• Acute myocardial infarction
• HOCM
• Increased ICP
• Aortic/mitral stenosis
• Pericarditis
• Cardiac tamponade
