INVESTIGATIONS & DIAGNOSIS
- Mood diary/symptom diary over 2-3 cycles
- Symptoms primarily occur in the luteal phase Symptoms ease with the onset of menstruation
- Must exclude other conditions:
-Depression
-Anaemia
-Thyroid issues
-IBS
-Endometriosis
TREATMENT PLAN
Mild:
- Lifestyle modifications
- Simple analgesia
- Patient information leaflets Moderate:
- New generation combined oral contraceptive pills (e.g. Yasmin: drospirenone + ethinyloestradiol)
- CBT
Severe:
- Above measures plus
- Consider SSRI antidepressants Follow up:
- Review in 2 months
- If no symptom improvement: a specialist review
KEY INFORMATION
- Severe form: Premenstrual Dysphoric Disorder (PMDD)
o Needs 5/11 distinct PMS symptoms & at least one mood alteration symptom required for PMDD diagnosis.
