Avoid Clarithromycin with statins:
- Stop simvastatin
- Reduce atorvastatin
Doxycycline:
- Contraindicated: pregnancy, breastfeeding, age <12 years
- Avoid retinoid interactions (risk of benign intracranial pressure)
- Monitor for thrombocytopenia, neutropenia, haemolytic anaemia
Fluoroquinolones:
- Avoid if there is a history of tendon issues
- Stop if tendinopathy develops (within 48 hours)
Amoxicillin interactions:
- Live typhoid vaccine: Avoid 3 days before/after
- Live cholera vaccine: avoid 14 days before, 10 days after
- Increased monitoring is needed with methotrexate – Increased rash risk with allopurinol
