Treatment Plan
Conservative:
- Self-care advice for viral URTI (3-4 weeks duration)
- Treat the underlying cause
Medications (for specific cases):
- URTI: Usually resolves on its own, but consider Doxycycline if systemically unwell – If Subacute, consider a trial of:
- Inhaled ipratropium
- Inhaled corticosteroids
- Oral prednisolone
- Antitussives (codeine)
- Eosinophilic bronchitis: Inhaled corticosteroid (+/- Montelukast)
- GORD: PPi
- Postnasal drip: Antihistamines/decongestants (If sinusitis coexists, use Inhaled CS/ Inhaled
Antihistamine/ Inhaled sodium cromoglycate
- For chronic refractory cases: Refer & treatment options include low-dose morphine,
Gabapentin, speech and language therapy
Specialist referral (routine):
- Non-response to treatment
- Undiagnosed aetiology
- Environmental exposure cases
Urgent Referrals:
- PE, tension pneumothorax, foreign body aspiration
