Treatment Plan
Conservative:
Strong indicators of a bacterial sore throat
0-1 = no antibiotics
2-3 = no antibiotics/delayed
4-5 = antibiotics/delayed
0-2 = no antibiotics
3-4 = antibiotics/delayed
- Fluids
- Analgesia, Difflam (or other local anaesthetics)
- Rest
Antibiotics (if indicated):
- First line: Penicillin V 5-10 days
- Second line: Clarithromycin, Erythromycin
Special situations:
- DMARD therapy with sore throat: stop medication, check FBC
- Post-chemo/radiotherapy: check FBC, consider admission
- Immunosuppressed: Check FBC, consider admission
Immediate admission if: – Clinical dehydration
- Breathing difficulty
- Stridor
Prognosis
- Self-limiting: resolves in 3-7 days (80%) in most cases
- Glandular fever: 1-2 weeks
