Treatment Plan

Mild cases:

• Low-intensity CBT

Moderate-severe cases:

• High-intensity CBT with ERP (Exposure Response Prevention)
• Medications: SSRIs or clomipramine

Medication considerations:

• Under 18: SSRIs only after a child/adolescent psychiatrist assessment
• Clomipramine contraindications: under 18, recent MI, mania, potassium issues, recent MAOI use

Referral criteria:

• Failed primary care treatment
• Severe OCD
• Age under 18
• Self-neglect

Prognosis

• Chronic course if untreated
• Waxing and waning pattern
